Health & Liability waiver Please fill out the waiver before arriving at the facility
As the participant/or parent/legal guardian of
I understand and appreciate the risks associated with the sport of baseball and related training activities. I am fully aware of the risks of injury involving catastrophic injury, paralysis, even death as well as other damages and losses associated with participation in baseball and related training activities.
I understand that it is the intent of Baseball Performance Center to provide for my safety and protection/the safety and protection of my child, in consideration for allowing me/my child to use this facility.
I hereby forever waive and release Baseball Performance Center, its owners, officers, employees, instructors, coaches, and independent contractors from all liability for any damages and injuries suffered by me/my child while under the instruction, supervision, or control of Baseball Performance Center.
Do you have any health conditions that restrict you from participating in physical activity?
MEDICAL RELEASE I hereby give my consent to Baseball Performance Center to provide emergency care and give authority to any hospital or doctor to render immediate aide that might be required at that time for me/my child’s health and safety.
By checking here, you submit that you have read, understand and agree to the above conditions, and that you have answered any question truthfully, to the best of your ability.
image RELEASE I hereby give my consent to Baseball Performance Center to use my/my child’s image in any form of media including print, television, internet, for advertisement, and/or promotional purposes.
As the participant/legal parent or guardian of the player named above, I hereby verify that I accept the conditions of the waiver and release and furthermore agree to/ permit my child to participate in the events provided by Baseball Performance Center.
